How To File a Workers’ Compensation Claim in Ohio

You can apply for workers’ compensation benefits in Ohio by filing a First Report of Injury (FROI) form. The form can be found at the BWC website: https://info.bwc.ohio.gov/, and you can complete the process by following the prompts. If you prefer, you may choose to download a written copy of the form and mail it to the BWC. There is also an option to apply by phone by calling the BWC directly at (800) 644-6292.

If you require medical attention for a workplace injury, call 911 right away. A delay in medical treatment after the accident occurs could make it more difficult for you to pursue a successful claim. Be sure to report all of your symptoms to your doctor and follow their treatment recommendations. It’s important to attend all necessary doctor’s office visits and rehabilitation appointments. If you have to miss an appointment, be sure to reschedule it right away.

You should report the injury to your supervisor as soon as possible. When you get a chance, use your cell phone camera to snap some photos of the accident scene. It’s also a good idea to collect the names and contact information of anyone who witnessed the accident.

Appealing an Ohio Workers’ Compensation Order if Your Claim Was Denied

It may take several weeks after you apply for workers’ compensation before you are notified of the outcome. The BWC issues a written order if your claim is denied, and you have 14 days from the date you receive the BWC order to file a Notice of Appeal (IC-12). You can also send a written document to the BWC. It’s important for your appeal to include all the necessary information.
